The oldest and most prestigious medical education facility in Russia is Siberian State Medical University (SibMed). By order of the Emperor Alexander II, the Imperial University at Tomsk was established on May 16, 1878. When it first began, Imperial University in Tomsk had only one faculty—the Faculty of Medicine—and was the only college in Russia's Asian region. A construction committee was established in March 1880. The governor of Tomsk, V.I. Mertsalov, was in charge of it. A.K. Bruni, an architect, created the construction plan. The construction process took over eight years to finish. On July 22, 1888, the University held its formal opening.

The Doctor of Medicine degree could be awarded by the university. 37 persons, 27 of them were University Faculty of Medicine graduates, successfully defended their theses during the pre-revolutionary era in order to acquire their degrees. Later on, many of them became professors. The Main Library was established to support educational and research activities for both teachers and students. There were 96,000 books gathered in 1888. The library at the time received a lot of donations of books. The library purchased a portion of the collection on its own dime. The study of medicine received a lot of resources.

The Tomsk Medical Institute had five faculties by 1945: dentistry, paediatrics, pharmacy, and medicine, sanitation, and hygiene. Due to demobilised soldiers, student groups started to reload. About 2,400 students began their studies at the Institute on September 1st, 1945. Between 1945 and 1950, almost half of the students had served in the military. Some of them went on to graduate school to finish their education.

SibMed is currently one of Russia's top three medical schools. It is recognised as Russia's top non-capital medical school and dominates among medical schools in the country's Asian region in terms of placements financed by the government. The Siberian State Medical University was the first Russian medical school to receive Skolkovo Technology Park accreditation in 2015. Since 2017, SibMed has held the title of Russia's sole medical flagship institution.
