What Is FMGE and Who Needs to Take It?
The Foreign Medical Graduate Examination (FMGE) is a screening test conducted by the National Board of Examinations in Medical Sciences (NBEMS) on behalf of the National Medical Commission (NMC). It assesses Indian citizens and Overseas Citizens of India (OCI) who obtained their primary medical qualification from a university outside India.
You must qualify FMGE to get registered with the NMC or a State Medical Council and practise medicine legally in India. For graduates from popular destinations such as Russia, Georgia, the Philippines, Uzbekistan and Kazakhstan, FMGE is currently the only route to medical practice in India. Is the 6-Attempts-in-3-Years Rule Real?
No. It was never an operative rule. Around December 2023, reports based on an NMC public notice claimed that FMGE candidates would get a maximum of 6 attempts within 3 years. The news spread widely and worried thousands of aspirants, but no such cap was ever notified by NBEMS in the FMGE information bulletins or applied to any exam session. Here is how the rumour compares with the current position:

What If You Don't Qualify FMGE in Your First Attempt?
If you do not clear FMGE in your first attempt, you can simply apply again for the next session by submitting a fresh application and paying the exam fee. There is no "three strikes" rule, no penalty for repeated attempts, and you will not be removed from the system because of past results.

How Many FMGE Attempts Do Most Students Need?
The FMGE attempt limit and the number of attempts a student actually needs are two different things. The second depends on your academic foundation, the quality of your university's training, your preparation strategy and the guidance you receive. Every extra attempt costs money and time, and it can dent your confidence.
FMGE has historically had a low pass rate. In the June 2026 session, fewer than 13% of candidates qualified. In a reply in Parliament in August 2026, the Minister of State for Health and Family Welfare stated that around 88% of the June 2026 applicants were repeaters, while the pass rate among first-time candidates was more than 35%. In other words, well-prepared first-timers do far better than the overall figure suggests. In our experience of counselling foreign medical graduates, many candidates clear FMGE within two or three attempts when they treat each attempt as a learning and revision cycle instead of repeating the same preparation method.
These factors matter more than the attempt number:
- Shorter gaps between attempts: Earlier, candidates waited about six months between sessions. With three sessions a year, the gap is now roughly four months, so you can correct your course faster.
- No negative marking: Since wrong answers do not cost marks, attempt all 300 questions. An educated guess is always better than leaving a question blank.
- A revised strategy: Analyse your weak subjects after every attempt and change your study plan. Repeating attempts without changing your approach rarely produces a different result.
What Are the FMGE Passing Marks?
Knowing the exam pattern and qualifying marks helps you set a clear target. Here is the breakdown:

The qualifying mark has stayed firm even under pressure. After the June 2026 result, FMGE candidates protested and demanded a lower cut-off. NBEMS set up an independent review committee, which did not grant compensatory marks or lower the passing criteria. FMGE Score Validity: Does It Expire If You Wait to Register?
No. NBEMS has not published any validity period for an FMGE pass. Unlike language test scores, which usually expire after two years, your FMGE result does not lapse.
Once you score 150 or above, you can use your FMGE pass certificate to apply for NMC or State Medical Council registration and for the Compulsory Rotating Medical Internship (CRMI) in India. There is no published deadline that forces you to register within a fixed time after passing.
Students often confuse the FMGE pass with the NMC Eligibility Certificate. Keep these two documents separate:

FMGE Eligibility Criteria 2026
To appear for FMGE, a candidate must meet the following requirements:
- Be an Indian citizen or an Overseas Citizen of India (OCI).
- Hold a primary medical qualification that is recognised for enrolment as a medical practitioner in the country where it was obtained.
- Have an NMC Eligibility Certificate or meet the NEET requirement applicable at the time of admission abroad.
- For students who joined after the NMC FMGL Regulations 2021: complete at least 54 months of study plus a 12-month internship at the same university, with the course taught in English.
- There is no upper age limit for appearing in FMGE.

Recent Changes in FMGE Exam Rules in 2026
For years, FMGE was held twice a year, in June and December. Following the student protests after the June 2026 result and the recommendations of the review committee, NBEMS announced in late August 2026 that FMGE would be conducted three times a year. An additional session was scheduled for 31 October 2026, and the session planned for January 2027 was moved to later in January or February 2027.
The committee also recommended a fixed difficulty mix for questions, allowing candidates to view their responses against the correct answers, and deferring video-based questions by one year. These changes apply to future sessions and do not alter the June 2026 results.
What Is NExT and Does It Affect Your Current FMGE Attempts?
The National Exit Test (NExT) has been proposed as a common licensing exam that would eventually replace FMGE. Here is the current position as of 2026:
- NExT has not been implemented, and no confirmed start date has been announced.
- If you are eligible for FMGE now, prepare for and appear in FMGE. Waiting for NExT can cost you valuable time.
NBEMS has not notified any cap on FMGE attempts, so you can keep appearing until you qualify. The practical time limit comes from elsewhere: under the NMC FMGL Regulations 2021, your foreign MBBS course and internship must be completed within 10 years of joining. The FMGE attempts themselves are not counted or capped.
No fixed deadline has been published for using an FMGE pass for registration. Most graduates start the Compulsory Rotating Medical Internship (CRMI) in India soon after the result, as it is needed before permanent registration. Check your State Medical Council's current requirements before you apply.
Your registration is based on the FMGE pass, not on how many attempts it took. The number of attempts is not a condition for CRMI allotment or NMC/State Medical Council registration. What matters more for future PG entrance and jobs is your performance in exams such as NEET PG after registration.
With FMGE now held three times a year, the gap between sessions is roughly four months. If your score was close to 150, appearing in the next session while your preparation is fresh usually makes sense. If you were far below, it can be wiser to skip one session, rebuild weak subjects and take full-length mock tests first.
